Marlette Regional Hospital Family Practices Earn PCMH Designation from Blue Cross Blue Shield of Michigan
Marlette Regional Hospital is pleased to share that Blue Cross Blue Shield of Michigan has designated Brown City Family HealthCare, Kingston Family HealthCare, Marlette Family HealthCare, and North Branch Family HealthCare as Patient-Centered Medical Home (PCMH) practices. The designation is a two-year certification.
With the PCMH model of care, our family practices establish medical teams that work in partnership with our patients to keep them healthy and monitor their care on an ongoing basis. This is accomplished through technology like our electronic medical record system that helps our care team track and monitor patients’ conditions and offer up-to-date recommendations for preventive care and chronic disease management. With an enhanced focus on educating our patients and keeping them informed, patients learn to better manage chronic conditions like asthma and diabetes.

At Marlette Regional Hospital, our healthcare offices have convenient office hours, same-day appointment availability, and 24-hour access to a clinical decision-maker by phone. Housed in the family healthcare offices, our Care Coordinator RNs work one-on-one with patients to educate them about disease processes and self-management at home.
