Funding for Navigator services was made possible by The Thumb Community Health Partnership (TCHP). The TCHP is composed of the hospitals, health departments, community mental health agencies, and a variety of area private-not-for-profit organizations. The Partnership’s goal is to work cooperatively together to meet the health needs, both physical and mental, of the residents of Huron, Lapeer, Sanilac, and Tuscola Counties. A recent grant award will fund seven navigators throughout the region. For more information on the Partnership, visit www.thumbhealth.org or call Kari Granz, the TCHP Program Coordinator, at 810-895-2918.
