Patient Health Planning Committee

The role of Patient Health Planners is to coordinate transitions of care for patients ready for the next phase of their recovery. Patient Health Planners also support a patient's readiness and understanding of transition to post-acute care for a healthier and safer patient experience.

HCECM, regional Patient Health Planners and representatives of various healthcare practice settings work together to improve care transitions throughout our east central Michigan region. Members of the committee collaborate to identify tools that can help improve transitions of care.

A tool that was developed by the committee is the HCECM Online Resource Directory. To provide members with an instrument to help coordinate continued patient care, HCECM spent months developing, researching and organizing over 6,500 resources to create an online directory. The intuitive program allows users to find doctors, hospitals and services more easily within a 14-county region. The directory was released in 2013 and is continuously updated.
